Submitting a Claim

For information about covered expenses, coverage limitations, policy maximums, and claim requirements, visit the Coverage page.

While on-contract, check your coverage level in your myEquity account before submitting a claim to ensure the contract has been processed and Crawford has the most up-to-date coverage information.

Claim submission methods

Crawford accepts Accident & Sickness benefits claims directly online, or by email or mail.

Submit your claim information, upload receipts and additional documents through the Equity-Crawford portal.

Send the completed PDF form(s) and scans/photos of supporting documents and receipts to equity@crawco.ca

Mail

Send the printed form(s) and copies of supporting documents and receipts to 100 Milverton Blvd – Suite 300 Mississauga, ON L5R 4H1

Scans and clear photos of required documents and receipts are both acceptable. Be sure to keep your original receipts for one year.

Medical / Paramedical Claim Form

For expenses related to physical therapy, medical expenses, and alternative medicine.

Practitioner Statement

PDF

May be required depending on your claim situation and should be submitted with your Medical/Paramedical Claim Form.

Accidental Dental Claim Form

PDF

For reimbursement of dental expenses when treatment is required as the result of an accident.

Vision Care Claim Form

PDF

For expenses related to vision care (i.e. eye exams, prescription glasses, contact lenses).

Income Replacement Benefits Claim Form

PDF

For income replacement claims when you are required to withdraw from a current or upcoming contract due to illness or injury. Additional supporting documentation is required.

Attending Physician Statement

PDF

For income replacement claims, this form must be completed by your physician and submitted with your Income Replacement Benefits Claim Form.
