Health & Insurance Benefits
One of the benefits of Equity membership is access to health and insurance coverage. Equity’s plans provide coverage for medical services and weekly disability, as well as accidental death & dismemberment insurance.

Eligibility
Equity members must be in good standing to be eligible for any health and insurance benefits. Login to MyEquity to view your current coverage eligibility.
On-Contract
Equity members on an Equity contract have access to accident & sickness benefits, which include medical services and weekly disability.
Non-members on an Equity contract have access to accident benefits only.
On-contract coverage levels are determined by the agreement or engagement policy and the company category (if applicable), and insurance premium.
Off-Contract
Equity members who have worked a total of eight weeks or more (excluding the Amateur, Festival, or Collective) during the calendar year are eligible for accident & sickness benefits, while off-contract in the following year.
Equity members who have NOT worked a total of eight weeks during the calendar year are eligible for accident benefits only, while off-contract in the following year.

Accident & Sickness Benefits
Administered by Crawford & Company
Accident coverage is related to an unforeseen and unintentional event causing injury. Sickness coverage is related to a medical condition that may have developed gradually or suddenly, but was not caused by an accident.
Accident & Sickness benefits include:
Weekly Disability Benefits
Weekly Disability Benefits provide for 60% of contractual weekly earnings up to a maximum of $1,500 per week, in the event a member’s disability is caused by injury or illness.
Accidental Death & Dismemberment
Administered by Chubb Life Insurance Company of Canada
Accidental Death & Dismemberment benefits provide varying levels of reimbursement for injury or loss of life, depending on if the member is off-contract, on-contract, or on-contract with an Extraordinary Risk Rider.
Making a Claim
Accident & Sickness claims can be submitted directly online or by sending a PDF version of the applicable Medical Services Claim Form by email or mail.
Scans and clear photos of required documents and receipts are both acceptable. Be sure to keep your original receipts for one year.
While on-contract, check your coverage level in your MyEquity account before submitting a claim to ensure the contract has been processed and Crawford has the most up-to-date coverage information.
Online
Submit your claim information, upload receipts and additional documents through the Equity-Crawford portal.
Send a completed PDF form and scans/photos of supporting documents and receipts to equity@crawco.ca
